中医教学门诊在本科毕业实习带教中的实践研究

  • 摘要:目的 对本科毕业实习带教中中医教学门诊的实际应用效果进行探讨。方法 选取江西中医药大学附属医院2020年6月至2022年9月实习的180名中医专业毕业实习生作为研究对象,随机等量分成实验组(中医教学门诊组)、对照组(传统门诊跟诊组)进行教学,每周2次。比较两组学员实习结束后理论和临床技能考核成绩,及其对教学效果的评价。结果 实验组学员在病案书写、辨证分析、理法方药运用、方解诠释,医患沟通等方面表现较对照组相比更优异,且差异具有统计学意义(P<0.05),问卷显示学员普遍认为中医教学门诊较传统教学门诊教学模式,更能提高学员学习兴趣与积极性,增强与患者的沟通能力,强化中医四诊运用的熟练度,提升中医临证逻辑思维能力,且两者差异具有可比性(P<0.05)。结论 中医教学门诊可以更好提升学员中医临床诊疗能力,值得进一步推广。                                                                                                                                                                                

  • Abstractobjective This paper probes into the practical application effect of TCM teaching and outpatient clinic in undergraduate practice teaching .Method A total of 180 TCM graduates who practiced in the Affiliated Hospital of Jiangxi University of Traditional Chinese Medicine from June 2020 to September 2022 were selected as the research objects and randomly divided into experimental group (TCM teaching outpatient group) and control group (traditional outpatient follow-up group) for teaching twice a week. The results of theoretical and clinical skills assessment and the evaluation of teaching effect were compared between the two groups. Result The students in the experimental group performed better than the control group in medical record writing, syndrome differentiation analysis, application of theory and prescription, interpretation of prescription, doctor-patient communication and other aspects., and the difference was statistically significant(P<0.05), The questionnaire showed that the students generally believed that compared with the traditional teaching outpatient teaching mode, the TCM teaching outpatient teaching mode can improve the students' interest and enthusiasm in learning, enhance the communication ability with patients, strengthen the proficiency in the application of the four diagnostic methods of TCM, and improve the logical thinking ability of TCM clinical syndrome, and the differences between the two are comparable(P<0.05). Conclusion TCM teaching outpatient clinic can better improve students' TCM clinical diagnosis and treatment ability, which is worthy of further promotion.

    毕业实习所见所闻对医学生的职业规划、职业素养有着重大影响[1],是一名医学生从理论到实践、课堂到病房,转化为一名临床医生的重要阶段。教学门诊是以能力为基础的教学(Competency-Based Education,CBE)模式,强调培养学员的综合临床诊疗能力。
