过敏性紫癜性肾炎的中医治疗

［摘要］总结皮持衡辨治过敏性紫癜性肾炎的经验。强调血分瘀热贯穿紫癜性肾炎病程始终，以清热解毒为治疗总纲，凉血化瘀贯穿全程，主张急性期从卫气营血及三焦辨治，认为慢性期仍余热未清、以肝脾肾三脏亏虚为主，提出紫癜性肾炎水肿应水瘀并治，肝脾肾亏虚者治宜补虚和络。

ABSTRCT: To summarize the experience of treating Henoch-Schonlein Purpura Nephritis with PI Chiheng. Emphasize points blood stasis heat throughout the course of the duration of purpura nephritis. In order to clear heat and detoxification for the treatment of the general outline, cool blood circulation throughout the whole process. It is suggested that the acute phase should be treated by weiqiyingxue and Trijiao. It is believed that residual heat is not clear in chronic phase, and the deficiency of liver, spleen and kidney is the main one. It is proposed that purpura nephritis edema should benefit water and remove blood stasis，the deficiency of liver, spleen and kidney should be treated by filling the deficiency and dredging the collaterals.

过敏性紫癜性肾炎简称紫癜性肾炎，是由过敏性紫癜所引起的继发性肾脏损伤，为免疫复合物介导性肾炎，该病所有年龄段均可发生，但好发于儿童，是儿童最常见的继发性肾小球疾病[1]。临床常表现为血尿、蛋白尿，部分患者可伴高血压及肾功能不全，肾外表现以皮肤紫癜为主，可有关节肿痛、胃肠症状、肌肉受累等表现，病情复杂，治疗棘手。中医对紫癜性肾炎没有系统的文献记载，根据其临床表现，可归属于“尿血”、“水肿”、“发斑”、“肌衄”、“葡萄疫”等范畴。现将导师辨治紫癜性肾炎的经验整理如下，以供同道参考和借鉴。
