益母生化汤治疗产后恶露不绝的临床研究

• 摘要:目的：研究益母生化汤运用在产后恶露不绝治疗中的临床效果。方法：分组依据为随机数字表法，将2019年1月—2020年6月我院接诊的86例产后恶露不绝患者分为对照组（纳入43例，行西医常规治疗）、研究组（纳入43例，行益母生化汤治疗），对两组的中医症候评分、恶露情况、术后3d宫底高度、综合疗效进行对比。结果：（1）治疗前，组间中医症候评分无明显差异，P>0.05；治疗后，研究组中医症候评分改善情况优于对照组，P<0.05；（2）研究组恶露持续时间、血性恶露持续时间、血性恶露量均少于对照组，且术后3d宫底高度低于对照组，P<0.05；（3）在综合疗效方面，研究组总有效率（93.03%）高于对照组（76.74%），P<0.05。结论：为产后恶露不绝患者进行益母生化汤治疗，能够改善恶露相关症状，促进子宫复旧，值得推广。                                                                                                                                                

• Abstract:Objective: To study the clinical effect of Yimu Shenghua Decoction in the treatment of postpartum lochia. Methods: according to the random number table method, 86 cases of postpartum lochia treated in our hospital from January 2019 to June 2020 were divided into the control group (43 cases were included in the conventional treatment of Western Medicine) and the study group (43 cases were treated with Yimu Shenghua Decoction). The TCM symptom score, lochia, the height of uterine fundus in 3 days after operation and the comprehensive efficacy of the two groups were compared. Results: (1) before treatment, there was no significant difference in TCM symptom score between groups, P > 0.05; after treatment, the improvement of TCM symptom score in study group was better than that in control group, P < 0.05; (2) lochia duration, blood lochia duration and blood lochia volume in Study Group were less than those in control group, and the height of uterine fundus was lower than that in control group 3 days after operation, P < 0.05; (3) in terms of comprehensive curative effect, the study group had better effect The total effective rate (93.03%) was higher than that of the control group (76.74%), P < 0.05. Conclusion: Yimu Shenghua decoction can improve lochia related symptoms and promote uterine involution, which is worthy of promotion.                                                                                                                                                                                                                

  产后恶露不绝是指产后子宫内浊液、淤血等淋漓不断，且伴有阴道出血，持续时间超过21d以上的临床症候，包括浆液恶露、白色恶露、血性恶露，分别可持续10d、20d、3d左右。西医认为产后恶露不绝与胎盘胎膜残留、羊水偏多、子宫内膜炎、子宫滋养细胞瘤、盆腔炎等因素有关，若是治疗不及时，容易引起晚期出血产褥感染、贫血等疾病，极易降低产妇生活质量，且目前主要采用止痛、抗感染等治疗方法，不过临床实践研究指出，疗效欠佳。中医认为产后恶露不绝主要由气血虚弱、饮食不节、冲任失司、七情内伤、饮食不节等因素造成，益母生化汤因兼具温经止痛、养血祛瘀之功效，对于治疗产后恶露不绝效果显著。
