  • 摘要:目的 探讨OSCE在儿科住院医师规范化培训年度考核中的应用价值。方法 对2018年一二年级规培学员进行OSCE年度考核及相同规培学员(二三年级)2019年进行OSCE年度考核,进行成绩分析及问卷调查、点评会。结果  2018年一年级学员各站成绩及考试总成绩明显低于二年级,有统计学意义,相同学员二年级比一年级各站成绩及总成绩明显提升,有统计学意义;2019年二三年级各站及总成绩差异不大,相同学员三年级(2019年)各站及总成绩与二年级(2018年)相比差异不大,无统计学意义。考官及学员对OSCE考核形式100%满意,设站合理,试卷难易程度适度。100%带教老师及住培管理人员肯定点评会发现教学及管理工作中的不足之处。结论 规培学员低年级学习成绩提升速度最快、高年级能达到规培标准,OSCE在年度考核中的应用可以全面考察学员在本年度专业知识及临床实践操作掌握情况,同时也可以综合测评基地住培管理及临床带教工作,值得推广应用。                                                                                                                                                                                

  • Abstract To explore the application value of OSCE in the annual assessment of standardized training for pediatric residents. Methods The 2018 first-year and second-year regular training students were assessed by OSCE, and the same regular training students (2019) were assessed by OSCE, and the results were analyzed, questionnaire survey and comment meeting were conducted. Results In 2018, the scores and total scores of the first-year students in each station were significantly lower than those in the second-year, which was statistically significant. The scores and total scores of the same students in the second-year were significantly higher than those in the first-year, which was statistically significant. In 2019, there was little difference between the stations and total scores of the second and third grades. Compared with the second grade (2018), there was no significant difference between the stations and total scores of the same students in the third grade (2019). Examiners and students are 100% satisfied with the OSCE assessment form, the station is reasonable, and the difficulty degree of the examination paper is moderate. 100% teachers and residents' training administrators will definitely find the deficiencies in teaching and management. Conclusion The academic performance of the students in the lower grades of regular training is the fastest, and the students in the higher grades can reach the standard of regular training. The application of OSCE in the annual examination can comprehensively examine the students' mastery of professional knowledge and clinical practice in this year, and can also comprehensively evaluate the management of residential training and clinical teaching in the base, which is worthy of popularization and application.

    客观结构化临床考试(Objective structured clinical examination,OSCE)是一种客观性较强、对医学生知识、技能和态度等临床胜任力综合评估的方法[1],但目前OSCE考试方式在儿科住院医师规范化培训中主要用于结业实践技能考核,但在结业考核中,即使考核发现了问题,也不利于基地对学员有针对性的再培训;我们将OSCE应用于儿科住院医师规范化培训年度考核中,目的是探讨住院医师规范化培训学员考核方式、及了解住培基地的管理及老师带教水平存在的问题,为深化儿科住院医师规范化培训教学改革,提高规培生积极性及评价儿科规培生临床胜任力提供依据。
