  • 摘要:目的:观察名老中医经验传承师带徒模式的脉诊快速诊法与教科书脉诊授学效果之间的差异,探讨更为有效的脉诊学习模式。方法:选取2020年8月至2022年1月来我院实习的中医学专业学生,随机分配到试验组40人和观察组40人。试验组学习脉诊快速诊法,并跟从张义明老师临床实习4周,每周不少于2天,其余时间按实习要求正常在病房轮转;对照组复习课本脉诊内容,在病房正常轮转4周。两组学生在4周前后分别进行脉诊理论考核、临床个案考核和自评表测试,并记录考核完成时间,统计分析学习前后各考核结果。结果:两组学生入组时各考核成绩、自评表得分及考核用时无统计学差异;4周后,试验组各测试结果明显优于对照组,且理论和个案考核用时明显短于对照组,有统计学意义。结论:脉诊快速诊法配合名老中医药专家临床带徒提高了脉诊学习的效果和效率,是脉诊教学的有效途径。                                                                                                                                                                                

  • Abstract To observe the difference between the rapid diagnosis of pulse diagnosis by famous old TCM experience inheritors and the teaching effect of pulse diagnosis in textbooks, and to explore a more effective learning mode of pulse diagnosis.Methods:Students majoring in Traditional Chinese Medicine from August 2020 to December 2021 were selected as interns in our hospital,40 people were randomly assigned to the experimental group and 40 people were randomly assigned to the observation group.The experimental group learned rapid diagnosis of pulse diagnosis, and followed Teacher Zhang Yiming for 4 weeks of clinical practice, no less than 2 days a week, the rest of the time according to the requirements of the practice in the ward rotation; The control group reviewed the content of pulse diagnosis in the textbook and rotated normally in the ward for 4 weeks.The students in the two groups were tested in pulse diagnosis theory test, clinical case test and self-assessment table test respectively 4 weeks ago and later. The completion time of the assessment was recorded, and the assessment results before and after learning were statistically analyzed.Results: There was no statistical difference between the two groups in the examination scores, self-assessment scores and examination time. After 4 weeks, the test results of the experimental group were significantly better than the control group, and the theoretical and case assessment time was significantly shorter than the control group, with statistical significance.Conclusion:The rapid diagnosis of pulse diagnosis combined with the model of clinical teaching led by famous old Chinese medicine experts can improve the effect and efficiency of pulse diagnosis learning, and is an effective way of pulse diagnosis teaching.

    脉诊是祖国医学在漫长的医学实践中创造出来的一种简便易行具有较强科学技巧的诊断方法。但“脉理精微,其体难症,……在心易了,指下难明”。目前的中医诊断学之脉诊教学主要以理论讲授为主,辅助于一些体验式练习[1,2]。但脉象的错综复杂使学习者很难领会与理解,其学习方法和学习效果一直不够理想。
